What you need with you for your 1st appointment before treatment is administered:
Insurance cards, if have insurance you must have copies @ time of your visit (clinic and HIPPA policy)
Patient Intake completed
Financial Policy & Terms of Acceptance (Read & Signed)
Medicare & Medica requires additional paperwork:
Patient must have read & signed before their appointment.
*Non-parent or guardian bringing minor child, authorization note from parent or guardian with signature to allow you to bring child to this clinic.
If all required paperwork is not completed prior to your visit, or if you have questions, please arrive 10-15 minutes before your scheduled appointment time.
If you need to cancel your appointment at our clinic, we kindly request a 24 hour notice.
